Informed Consent for Structured Questionnaire

Hello,
The Kenya National Commission on Human Rights (KNCHR) is conducting mapping of organizations of and for persons with psychosocial and intellectual disabilities in order to create a strong advocacy platform on monitoring psychosocial and intellectual disabilities in Kenya and also to facilitate the involvement of these organizations in the monitoring process under Article 33 (3) of the CRPD. Your organization has thus been identified to take part in this mapping.

Benefits of the study:
The information you provide will enable the Commission to create a strong advocacy platform on monitoring psychosocial and intellectual disabilities in Kenya.

Risks of the study:
There are no direct or indirect risks in your participation in this interview.

Confidentiality:
Your identity and responses shall only be used for the purposes of this mapping. The Commission at the end will seek your consent to collate, compile and publish your responses. The information you provide will contain no details that would allow individual subjects to be linked to specific statements. Please answer the questionnaire as accurately and truthfully as possible.

DEFINITION OF TERMS
Persons with disabilities: Article 1 of the CRPD defines persons with disabilities include those who have long-term physical, mental, intellectual or sensory impairments which in interaction with various barriers may hinder their full and effective participation in society on an equal basis with others.

Psychosocial disability: disability that arise from barriers to social participation, experienced by people who have or who are perceived to have mental conditions that tend to be episodic, for instance bipolar and schizophrenia.

Intellectual disability: means a significantly reduced ability to understand new or complex information and to learn and apply new skills (impaired intelligence). This results in a reduced ability to cope independently (impaired social functioning), and begins before adulthood, with a lasting effect on development. Examples include; downs syndrome, autism.

Organizations of persons with disabilities: means associations or societies that are led, directed and governed by persons with disabilities and in which a clear majority of the membership is recruited from among persons with disabilities themselves with the collective aim of acting, promoting, pursuing, or defending rights of persons with disabilities (General Comment No. 7 of the CRPD para 11).

Organizations for persons with disabilities: means associations, groups, non-governmental organizations and/or societies formed for the purposes of rendering services to (or) on behalf of persons with disabilities (General Comment No. 7 of the CRPD para 13).

Cross-disability organizations: represent all or some of the wide diversity of impairments (General Comment No. 7 of the CRPD para 12 (b)). In this case, these organizations will be those that deal with both psychosocial and intellectual disabilities.
